The application is open and will close on August 22, 2026. Nurses are paid between the minimum salary of $44.56 to the maximum salary of $60.98. Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b) (i) or (ii), as applicable.

The job description is as follows:

• You will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families.
• You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance.
• You will provide safe, quality patient and family centered care while reflecting the shared vision and values.
• Reporting to the Care Manager, you will play a key role in coordinating and directing care for clients within the Home Care program, using your strong assessment, organizational, leadership, conflict resolution, and negotiation skills.
• You will work with a high degree of autonomy and confidence, often with limited onsite support, while collaborating with interdisciplinary teams, clients, families, and community partners to develop, implement, and continuously evaluate individualized care plans.
• Your excellent verbal and written communication skills will be essential in facilitating care planning discussions, client and family education, resource coordination, and timely referrals to appropriate services.
• Leveraging your expertise in wound care, palliative care, and home parenteral therapy, you will ensure clients receive high-quality, person-centered care that supports their health and independence.
• You will also have the opportunity to provide direct care as needed and adapt to changing priorities across assignments.
• For applicants to be considered, they must have the following:
• Minimum one (1) year of Home Care Case Manager experience within the last five (5) years.
• Experience with Connect Care, care planning, and Resident Assessment Instrument (RAI) Assessments.
• Demonstrated knowledge of chronic disease management, palliative care, geriatrics, and community health. Proven ability to develop, implement, monitor, and evaluate individualized care plans to support client-centered outcomes.
• Strong understanding of medication management, treatment plans, and interdisciplinary care coordination.
